Clearance change of theophylline during initial short period of smoking withdrawal in patients with obstructive airway diseases





Abstract
Clearance changes of theophylline during initial short period after smoking withdrawal were observed in the patients with obstructive airway diseases(n=10) and compared with those of non-smoking patients as a control group(n=7). Serum theophylline concentrations were analysed with Abbott TDx^(¢ç). System and clearances were calculated with Abbottbase^(TM) Pharmacokinetic Software program using Bayesian analysis.
Initial clearance of smoker group was significant larger than that of non-smokers(67.48¡¾19.47§¢/min/§¸ vs 35.06¡¾5.50§¢/min/§¸). The clearances in smoker group were linearly(Y=1.0173-0.0317X, r=0.8772) reduced in 7 of 10 subjects and approached almost to the initial value of non-smokers, but no significant changes were found in non-smoker group. In smoker group, the reduction % of theophylline clearance during first 2 weeks after stopping smoking was estimated to be about 43%.
It is concluded that the clearance of theophylline might be normalized within first few weeks after stopping smoking in the hospitalized patients with smoking history, therefore, careful and frequent serum level monitoring should be done during first 2-3 weeks of hospitalization to make sure safe and therapeutic drug responses.
